Rapid Response

This service is especially designed to alleviate CHC pressures through lack of capacity of providers to take on CHC packages the aims are to ensure:

  • Effective response to the request for a CHC package
  • Rapid response to the requests for package provision
  • Assessment within 24 hours
  • Care plan and full risk assessment in place within 48 hours
  • Responsive office team allocated consultant for Rapid Response
  • Office available from daily 5.00am-10.00pm and 24 hours out of hours team
  • Monthly reports
  • Effective evaluation of all services
  • Weekly time schedules for all clients
  • Rapid response to changes in care
  • Service will liaise with MDT as required
